Application for Graduation ( PDF | 1.1Mb)

 In order to officially graduate from your degree program, you must complete this form. The deadlines vary for fall and spring graduation. Please contact Student Services or consult the semester calendar located in the schedule for the specific deadline date.

Audit Request Form ( PDF | 100Kb)

 A student must complete this form in order for the campus registrar to officially audit your transcript for degree eligibility.

 Changing your major? Transferring to a different school? You need to complete this form to make it official.

 You can print and fill out this form to register for campus classes.

Transcript Request Form ( PDF | 67Kb)

To receive an official sealed transcript, you must complete this form and submit a $7.00 fee for regular processing which takes 7-14 business days to process. An additional $5 can be submitted for priority processing which is a 1-3 business day turn-around.
